Empowered by Everyone to Transform Lives. **Overview*** Provide direct care to and supervision of individuals served, ensuring their health, safety, and well-being.* Implement Individual Service or Treatment Plans (ISPs/ITPs), Behavior Support Plans (BSPs), and program routines.* Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.* Facilitate activities that promote skill development, independence, recreation, and community engagement.* Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.* Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.* Safely transport individuals to appointments, activities, or community outings as required.* Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.**Additional Information**MyPath and its Operating Companies provide specialized services and dignified care for children, adolescents and adults with special needs.
