Working within the Collaborative Care Model (CoCM) youll have the opportunity to make a meaningful difference in patients lives by providing evidence-based interventions coordinating care with medical providers and psychiatric consultants and helping patients achieve healthier more fulfilling lives.

You may be a great fit if you have:

Masters-level licensure in a behavioral health field (LCMHC LCSW LMFT) or associate-level licensure (LCMHC-A LCSW-A LMFT-A)

Experience with mental health screening assessment and treatment planning

Strong communication and relationship-building skills

Comfort working with patients both in person and by phone

Familiarity with evidence-based behavioral health interventions and psychopharmacology

Strong organizational and documentation skills

A compassionate approachable and patient-centered mindset
