The Licensed Practical Nurse (LPN) provides direct nursing care to residents while collaborating with Registered Nurses, physicians, and other healthcare professionals to ensure the highest standards of care. This role supports both long-term care residents and rehabilitation patients working toward recovery goals.

Responsibilities include administering medications and treatments as prescribed by physicians, monitoring and documenting residents' health status, reporting changes in resident condition to the supervising RN or physician, assisting with activities of daily living (ADLs) as needed, collaborating with the rehabilitation team to support recovery plans, maintaining accurate and timely documentation in accordance with facility policies and regulations, providing emotional support and education to residents and families, and maintaining a safe and clean environment while following infection control protocols.

Qualifications include a current Licensed Practical Nurse (LPN) license in good standing, strong clinical skills including medication administration and wound care, excellent communication and interpersonal skills, ability to work effectively within a team environment, compassionate, patient-centered approach to care, and BLS/CPR certification required.
