Park Avenue Nursing Home in Meadville, PA, is seeking an enthusiastic and compassionate PRN Physical Therapist Assistant (PTA) to join our rehabilitation team!

What You'll Do• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist
• Implement individualized treatment plans to improve mobility, strength, balance, and functional independence
• Monitor resident progress and communicate changes to the supervising Physical Therapist
• Collaborate with nursing staff and other healthcare professionals to provide exceptional resident care
• Educate residents and caregivers on therapeutic exercises, mobility, and safety techniques
• Maintain accurate and timely clinical documentation using Net Health
• Help create a positive, encouraging rehabilitation experience for every resident
Qualifications• Graduate of an accredited Physical Therapist Assistant (PTA) program
• Active Pennsylvania PTA license in good standing required
• Pennsylvania Indirect Supervision License preferred
• SNF/LTC experience preferred
• Experience with Net Health preferred, but training can be provided
• Strong communication, organizational, and interpersonal skills
• Compassionate, dependable, and committed to providing quality resident care
