This is a great position for someone with strong case management experience, leadership experience, and a passion for delivering exceptional service and fostering strong referral relationships. We do this by hiring only those caregivers who can build real connections with their clients and by offering them the community of support and recognition they deserve. A day in the life of a Case Manager In this role you will work with prospective and current clients, their families, and other professionals involved in their care to ensure they have a positive experience from the start of their home care services. This will be done by: Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments Overseeing the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans Providing regular, proactive communication to all parties involved in each client's care Revising care plans as needed and monitoring the quality of services provided Developing strong and trusting relationships with Tribute's clients and their families In addition to building relationships with clients, you will also build relationships with community referral sources. We are seeking a candidate who is creative and resourceful and eager to promote and grow a service they will be proud of. This role requires regular travel throughout the greater Chicago area, including client visits and meetings with referral sources, as well as working from Tribute's Skokie office on Wednesdays. The position offers flexibility to work remotely at other times.
