Take Your Therapist Home As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program. Rivetus Rehab is a specialized rehabilitation company serving the geriatric Post‑Acute community since 1998. Our perspective is unique because we provide continuity of care across Skilled Nursing, Home Health, and Outpatient settings, all managed by licensed clinicians. This position is primarily in the home health setting with dedicated time and caseload support for the skilled nursing facility (SNF). Opportunity to expand into other settings. Facility Location Belle Fountain – This hybrid role provides facility‑based therapy services and local home‑care therapy services. Facility services are paid hourly; home‑care services are paid per visit. Schedule Monday–Friday, Full Time. General Summary In this role you will provide physical therapy services to patients with physical dysfunction, including assessment, treatment planning, and implementation of treatment plans. You will participate in patient care conferences, program development, and screenings as appropriate. You will supervise PTAs and aides as assigned and complete documentation that meets quality, continuity, and reimbursement requirements. Your Responsibilities Conduct physical therapy screenings and evaluations to determine patient physical status. Provide physical therapy treatment consistent with professional and ethical standards of the APTA. Develop and implement treatment plans using strengthening, modalities, and retraining to aid patients in reaching independence. Provide direct patient care in accordance with productivity standards using graded activities to restore physical and functional abilities. Check, instruct, assign, and review the work of PTAs and aides. Document work to maintain quality of the medical record and meet reimbursement guidelines. Participate in patient care conferences as requested. Participate in CQI activities to ensure continuous quality of care. Promote current standards of clinical practice through continuing education and in‑service programs. Coordinate and/or participate in in‑service programs for internal or external groups. Participate in new program development. Attend company meetings and additional activities to support departmental operations. Required Qualifications Successful completion of an accredited Physical Therapy program. Licensure as required by the State of Michigan. Valid driver’s license. Skills and Abilities Demonstrate professional judgment and initiative when analyzing problems and recommending solutions. Show initiative and flexibility in the work environment. Respond effectively to the needs of patients and their families. Physical ability to bend, position, transfer, and lift patients weighing from 80 to over 300 pounds, with the use of recommended lifting devices. Why Rivetus Competitive pay.
