LPN - Position DescriptionComfort Keepers, Plymouth, MIAdministrative Responsibilities:Monitor and report changes in patient symptoms or behavior including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points and reports for each shiftCollaborating with RN Administrator, physicians, or specialists regarding patient careEvaluate the patient and home situation to determine what health teaching will be required, if additional services are needed and the frequency of this serviceDocument services and education provided to the patient, using an Electronic Medical Record (EMR) application, and patient scheduling. MUST be able to adhere to strict deadlines for Start of Care (SOC), revisits, re-certifications, and dischargesParticipate in Case Conferences, In-Services and mandatory Staff Meetings as neededPatient Care Services Performed:Take patient vital signs as requiredSuction via trach per the patient's plan of careTube feed administration, medication administrationVentilator Maintenance, and pulse ox readings as recommendedCOPD- Congestive Heart Failure (CHF)-With RN Administrators guidance: Monitor and provide education on daily weight logs, fluid restriction education, medication management, vital sign teaching and reporting.
