The Role

We are seeking an empathetic, proactive, and organized Social Services Manager to lead our resident advocacy and social services program at Martha Franks Retirement Community!

In this key leadership role, you will serve as the primary advocate for our residents and their families. You will conduct psychosocial assessments, coordinate care plans, facilitate smooth admissions and discharge transitions, and provide emotional and practical support to ensure our residents' physical, mental, and social needs are met. Key Responsibilities

• Resident Advocacy & Psychosocial Care: Complete initial and ongoing psychosocial assessments, identify social and emotional needs, and develop individualized social service care plans for each resident.

• Admissions & Discharge Planning: Coordinate smooth transitions into the community and manage safe, comprehensive discharge planning in collaboration with residents, families, and interdisciplinary clinical teams.

• Family & Resident Support: Serve as a compassionate point of contact for residents and family members, offering guidance, counseling resources, and assistance with adjustments to senior care living.

• Interdisciplinary Collaboration: Participate actively in care plan conferences, resident assessment meetings, and quality assurance reviews alongside nursing, therapy, and administrative leadership.

• Community & Resource Liaison: Connect residents and families with external community resources, Medicare/Medicaid navigation, legal assistance, hospice support, and specialized mental health services as needed.

• Regulatory Compliance: Ensure all social service documentation strictly adheres to state and federal healthcare guidelines, facility policies, and confidentiality standards (HIPAA).

Qualifications

• Education: Bachelor’s degree in Social Work (BSW), Human Services, Psychology, or a related healthcare field required (Master’s in Social Work / MSW or LSW preferred).

• Experience: Minimum 2+ years of social services or discharge planning experience in a Senior Living, Skilled Nursing Facility (SNF), Assisted Living, or Long-Term Care environment.

• Knowledge: Strong understanding of Medicare, Medicaid, long-term care insurance, community resources, and state healthcare regulations.

• Interpersonal Skills: Exceptional active listening, conflict resolution, crisis management, and written/verbal communication skills.
