Health Improvement Coordinator

The Iowa Department of Health and Human Services, Division of Public Health, Bureau of Chronic, Congenital and Inherited Conditions is seeking a Health Improvement Coordinator to join our team. This role plays a central part in advancing the mission of the Cardiovascular Health Program, with a major focus on leading and expanding the Iowa Cardiovascular Collaborative, a statewide network of partners working together to improve cardiovascular health outcomes for all Iowans. The position is responsible for guiding, implementing, and evaluating strategies aimed at reducing chronic disease and addressing related social and community needs. It provides technical expertise to healthcare partners, community organizations, contractors, and stakeholders across Iowa, ensuring alignment with CDC guidance, federal cooperative agreement requirements, and evidence‑based best practices in chronic disease prevention and management. Key Responsibilities Coordinate, expand, and maintain the statewide Cardiovascular Collaborative to advance evidence‑based prevention and management strategies. Align Collaborative activities with CDC cooperative agreements, state priorities, and emerging evidence. Build and manage a strong network of multidisciplinary partners, ensuring meaningful engagement across public health, healthcare, community organizations, and social service sectors. Organize and maintain the Collaborative’s structure, work groups, initiatives, and communication channels. Translate cooperative agreement requirements into clear expectations, guidance, and actionable tasks for Collaborative members. Oversee collection, tracking, and documentation of action plans, progress, and outcomes to demonstrate impact and guide improvement. Prepare and deliver presentations, trainings, and updates to partners, internal teams, and external stakeholders. Support initiatives and activities for chronic disease programming through content development for work plans, budgets, and progress reports. Serve as a subject matter expert for assigned work groups and statewide initiatives. Provide technical assistance to partners implementing chronic disease prevention and management strategies. Promote evidence‑based programs and interventions that advance chronic disease prevention and management. Collect, analyze, and report program performance data, identifying trends, gaps, and opportunities for improvement. Participate in program evaluation activities and use findings to guide program enhancements. Support program performance management through development, monitoring, and evaluation of performance measures. Facilitate continuous quality improvement activities with internal teams and external partners. Plan, facilitate, and participate in meetings with program partners, contractors, and stakeholders. Develop and disseminate agendas, meeting minutes, action items, and communication updates. Prepare success stories, health impact statements, and summaries of program accomplishments. Ensure adherence to federal and state guidance, cooperative agreement requirements, and agency policy. Provide regular updates and reports to the Program Manager and Section Supervisor. Selectives 535 Public Health – 6 months experience, 12 semester hours, or a combination of both related to improving, maintaining, and protecting the general condition of health within society, either on a personal, institutional, organizational, or community basis. Experience may be gained from direct provision of health care or from other ancillary programs and support mechanisms such as government public health departments. Minimum Qualification Requirements Graduation from an accredited college or university with major coursework in nursing, a human services or health‑oriented field, education, or nutrition and three years of professional experience in these areas. Masters degree in nursing, public health or science, a human services or health‑oriented field, or education, may be substituted for one year of the required experience. Certification as a paramedic and the equivalent of three years of full‑time experience as a paramedic. An equivalent or combination of the required education and experience, totaling seven years, will be qualifying.
