As an Advanced Practice Clinician, you'll play a key role in delivering personalized care that improves quality of life and health outcomes. Key Responsibilities Complete comprehensive patient assessments, including: Review of medical history and medications Physical examinations and vital signs Appropriate screenings and point-of-care testing Support members through short-term sub-acute rehabilitation stays Maintain thorough and accurate clinical documentation, including ICD-10 coding Partner closely with interdisciplinary care teams to coordinate treatment plans Lead discussions around advance care planning and serious illness care Educate and counsel patients, families, and caregivers You’ll be recognized and rewarded for your contributions while working in an environment that supports career development and professional advancement. Required Qualifications For Nurse Practitioners: Current ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care For Physician Assistants: Active NCCPA certification and applicable state licensure Additional requirements: Unrestricted state license (or ability to obtain before start date) Valid driver's license with reliable transportation Active DEA license/prescriptive authority, or ability to obtain after hire in accordance with state requirements Ability to establish a collaborative practice agreement when required Ability to work in a variety of clinical settings and perform essential job functions Preferred Qualifications One or more years of APC experience, preferably in long-term care Experience in geriatric care, senior living, home care, or long-term care environments Familiarity caring for patients with complex medical, behavioral, social, or functional needs Knowledge of chronic disease management and geriatric medicine Experience facilitating advanced illness and end-of-life conversations Comfort using electronic medical records and healthcare technology Compensation: Hourly compensation for this position typically ranges from $53 to $79 per hour , based on factors such as location, experience, education, and other job-related considerations.
