Registered Nurse - Hospice Care

Assignment Duration

This is a 13-week contract position.

Schedule

5 days per week, 8:00 AM to 4:00 PM, with every other weekend. Comp days available during the week.

Job Description

As a Registered Nurse specializing in Hospice Care, you will be responsible for delivering compassionate care to patients and their families. This role involves patient assessments, developing and implementing care plans, and ensuring compliance with hospice regulations and insurance guidelines. You will also be responsible for case management, interdisciplinary communication, and participating in performance improvement initiatives.

Responsibilities

• Conduct patient assessments to determine appropriateness for hospice care.
• Develop and evaluate plans of care for patients and families.
• Maintain visit productivity standards and ensure proper documentation.
• Utilize age-appropriate teaching methods and document patient education efforts.
• Manage case responsibilities and prioritize care needs effectively.
• Participate in IDT meetings, providing updates on patient progress and care plans.
• Ensure a safe environment through infection control and safe medication management.
• Engage in staff meetings and contribute to performance improvement activities.
• Mentor and orient new employees, fostering a collaborative work environment.

Required Experience / Certifications / Licensure

Must hold a valid RN license and have experience in hospice or palliative care. Strong knowledge of insurance reimbursement and hospice regulations is preferred.
