Job Description

Job Summary:
Provide skilled physical therapy assistance to patients under the supervision of licensed physical therapists. Contribute to patient recovery plans and ensure high-quality rehabilitative care within a skilled nursing facility setting.

Responsibilities:

• Assist in implementing individualized treatment plans for patients recovering from illnesses or surgeries.
• Monitor and document patient progress, reporting any concerns to the supervising therapist.
• Administer therapeutic exercises, modalities, and activities as directed.
• Maintain cleanliness and safety of treatment areas and equipment.
• Collaborate with healthcare team members to optimize patient outcomes.
• Follow all facility policies, procedures, and confidentiality guidelines.

Qualifications:

• Hold an active California Physical Therapy Assistant license.
• Possess previous SNF experience and familiarity with NetHealth (Rehab Optima) software preferred.
• Demonstrate strong clinical skills in rehabilitative therapies.
• Exhibit excellent communication and teamwork abilities.
• Maintain current CPR certification.
• Be detail-oriented with a commitment to patient care and safety.
