LPN Med Care Manager

The LPN Medication Nurse provides licensed nursing support to ensure safe and accurate medication administration, health monitoring, and proper clinical documentation in a senior living environment. Key Responsibilities: Administer medications and treatments following physician orders and the Six Rights of Medication Administration. Accurately document in MAR/TAR in real-time and support care plan updates. Monitor resident symptoms, report adverse reactions or refusals promptly, and observe physical, emotional, or cognitive changes. Maintain medication carts, conduct narcotic counts, and ensure compliance with regulatory standards. Conduct Service & Health Updates (SHUs) and transcribe physician orders. Qualifications & Skills: Active LPN license in good standing with CPR and First Aid certification. Strong assessment, observation, and documentation skills. Experience in senior care or medication administration is preferred.
