Home Health Physical TherapistA Home Health Physical Therapist provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, and functional independence. This role includes direct patient care as well as completion of federally required OASIS assessments to support clinical outcomes, care planning, and regulatory compliance.Key Responsibilities:Conduct comprehensive in-home physical therapy evaluations and functional assessmentsComplete and maintain accurate OASIS documentation at required admission, reassessment, recertification, and discharge pointsDevelop individualized treatment plans based on patient condition, functional limitations, and home environmentProvide skilled interventions including therapeutic exercise, gait training, balance training, and functional mobility trainingAssess home safety risks and recommend adaptive equipment or environmental modificationsEducate patients and caregivers on safe mobility techniques, fall prevention, and home exercise programsMonitor patient progress and adjust care plans based on clinical outcomesCollaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient careEnsure timely, accurate documentation in compliance with Medicare and agency standardsTravel to patient homes within assigned service areasOASIS Responsibilities Include:Functional mobility and ADL assessmentFall risk and safety evaluationPain and cognitive status documentationIdentification of clinical changes impacting plan of careSupporting reimbursement and quality reporting requirementsQualifications:Doctor of Physical Therapy (DPT) or equivalent degree from an accredited programCurrent state Physical Therapist licensureCPR certification (or ability to obtain)Valid driver’s license and reliable transportationExperience with home health care and OASIS documentation strongly preferredSkills & Competencies:Strong clinical reasoning and independent decision-makingProficiency or willingness to learn OASIS documentation standardsExcellent communication and patient education skillsStrong organizational and time management abilitiesAbility to adapt care to varied home environmentsCompassionate, patient-centered approach to rehabilitationWork Environment:Patient homes in assigned geographic territoryRegular travel required throughout the workdayFlexible scheduling based on patient needs and caseloadPhysical Requirements:Ability to safely assist patients with transfers and mobilityFrequent standing, walking, bending, and light-to-moderate liftingAbility to work in non-clinical, variable home environmentsRole Objective:To enhance patient independence and quality of life by delivering skilled physical therapy in the home setting while ensuring accurate OASIS documentation that supports high-quality, compliant, and outcome-driven care.
