In-Home Health Physician (Per Diem)

Signify Health, a CVS Health company, is seeking a Per Diem Physician to perform in-home health evaluations. You will engage with members in their homes, review medical history, conduct a physical assessment, and coordinate with health plans to connect patients with the right care.

The role requires MD or DO with an active license, the ability to travel locally and within licensed states, and the option to conduct virtual visits as needed.
