Key Responsibilities
- Provide occupational therapy services to patients across a multi-county home health territory
- Meet a minimum productivity expectation of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day
- Utilize HCHB (Homecare Homebase) for documentation and care coordination
- Deliver patient-centered care within assigned home health territory spanning multiple counties
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Washington
- Minimum of one year of recent home health OT experience
- Proficiency with HCHB (Homecare Homebase) documentation platform
- Ability to travel and provide services across a large multi-county territory
Preferred Experience / Skills
- Familiarity with home health operations across diverse geographic service areas
- Strong time management skills to meet daily documentation deadlines
- Experience managing a high-volume caseload in a home health setting
