Physical Therapist

Job Summary:

The Physical Therapist is responsible for providing skilled physical therapy care to patients, including comprehensive assessments, development and implementation of individualized plans of care, and coordination with interdisciplinary teams. The role involves supervising support staff, ensuring compliance with regulatory standards, documenting clinical outcomes, and promoting rehabilitation services. The Physical Therapist is expected to maintain up-to-date knowledge and engage in community outreach to enhance service delivery. Responsibilities:

• Monitor and adjust patient visit schedules to optimize productivity and meet patient needs.
• Perform comprehensive assessments including psychosocial, educational, safety, and risk factors.
• Develop and document individualized plans of care with patient-specific goals and interventions.
• Provide skilled physical therapy treatments and patient/caregiver education.
• Coordinate patient care with physicians, nurses, caregivers, and other rehabilitation team members.
• Supervise and direct activities of PT assistants, students, and home care aides.
• Complete and maintain required clinical documentation in compliance with federal, state, and organizational standards.
• Ensure accurate billing and insurance documentation and address coverage inconsistencies.
• Participate in training programs for staff, volunteers, and community groups.
• Maintain and enhance professional knowledge and standards in physical therapy.
• Engage in marketing and promotion of rehabilitation services.
• Perform various patient health assessments and procedures such as PT/INR, blood pressure, wound care, and staple/suture removal.
• Communicate schedule changes promptly to supervisors and schedulers.
• Complete OASIS documentation and collaborate with Quality Improvement staff to ensure accuracy.
• Follow agency protocols for documentation and visit management using McKesson systems.
• Assess and coordinate Home Care Aide services as needed.
• Coordinate safe patient discharges and transfers with proper documentation.
• Report patient incidents timely to Quality Improvement specialists.
• Participate in peer review audits and provide constructive feedback.

Required Skills & Certifications:

• Licensed Physical Therapist in the state of practice.
• Comprehensive knowledge of federal and state regulations, organizational policies, and APTA guidelines.
• Proficient in clinical assessments, plan of care development, and skilled physical therapy interventions.
• Strong documentation and communication skills.
• Experience coordinating interdisciplinary care and supervising clinical staff.
• Ability to use electronic medical record systems and billing documentation accurately.
• Minimum of 2 years of relevant physical therapy experience preferred.

Preferred Skills & Certifications:

• Experience with home health or hospice physical therapy.
• Knowledge of marketing and community outreach in rehabilitation services.
• Familiarity with McKesson or similar healthcare software systems.
• Advanced training or certifications in specialized physical therapy techniques.

Special Considerations:

• Compliance with regulatory standards including federal, state, and agency-specific guidelines.
• Ability to manage patient schedules proactively and communicate changes promptly.
• Participation in peer reviews and quality improvement initiatives.
• Occasional requirement to supervise or coordinate care with home care aides and other support personnel.

Scheduling:

• Work schedule determined by patient visit demands and agency policies.
• Visits and documentation must be completed in a timely and efficient manner.
• Coordination with schedulers to manage changes and maximize productivity.
