Pharmacist - Hematology/Oncology Specialist

Job Overview Under the Department of Pharmaceutical Services, the Ambulatory Clinic Pharmacist provides pharmaceutical care to patients at all ambulatory care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Ambulatory Clinic Pharmacist performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Ambulatory Clinic Pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff. The pharmacist will be based primarily in the ambulatory care setting managing a specific patient population. Depending on the assigned shift and site, the pharmacist may also oversee pharmacy technicians to facilitate insurance authorizations for medications, as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The pharmacist may also be responsible for the oversight of clinical activities, delivery of services, and the Patient Management Program. Oversight of the Patient Management Program is usually limited to when the pharmacist is working a shift in collaboration with the UCSF Specialty Pharmacy.
