Description Case Manager Bilingual Department El Centro Created 10/7/2021 Reports to Hispanic Outreach Manager Revised 8/21/2024 FLSA Status Non-exempt EEO - 1 Service Workers Position Type Full Time Travel Regular travel within assigned counties Work Hours Mon – Fri, 8:00 am – 5:00 pm Positions Supervised None Summary The Case Manager position provides quality case management services through a strengths-based approach to youth, adults, and their families, in support of maintaining residence in a family environment, learning independent living skills, attending, and succeeding in educational endeavors, and preventing or reducing hospitalizations. Essential Functions Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions. Provides direct case management services to child and adult clients and to their families, usually on a weekly basis, in an effort to maintain stability and residence in a family environment. Meets or exceeds the billable hour standard of client contact hours set for position. Assists clients and their families to identify, secure, and maintain community resources. Provides and coordinates necessary referrals and delivery of services among schools, community agencies, and within the organization to ensure that client is receiving necessary services. Works in partnership with clients and other vested individuals to design and implement a comprehensive and individualized plan of care based on a strengths assessment. Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and CrossWinds standards and requirements. Submits daily appointment sheets, progress notes, treatment plans, client status reports, timesheets, and all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements. Develops knowledge of community resources and how to access these services for the benefit of clients. Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, Center staff members, and visitors. Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements. Skills & Abilities Fluency in both English and Spanish (written and oral); ability to work with persons with serious emotional disturbance and/or severe and persistent mental illness; ability to work independently and as a member of a therapeutic team; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations. Education & Experience Associate’s degree plus two years’ experience in the health and human services field OR four years’ experience in the health and human services field is required. Bachelor’s degree in psychology, social work, or counseling is preferred. Certifications & Licenses Must complete Case Management Training and provide certificate within 6 months of hire. Computer Skills Computer experience in Microsoft Office programs and basic computer fluency. Work Environment This job operates in a community setting, as well as in a professional office setting. This position primarily works with clients out in the community and within the agency’s residential facilities. This role uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. Language skills are required, including the ability to read, write, comprehend, and speak the English language. This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.
