ABOUT THE JOB
The Nurse Practitioner provides personal, direct primary care to patients with acute and/or chronic illnesses in collaboration with physicians. The Nurse Practitioner assesses individual/families using history, physical exam and appropriate diagnostic tests/x-rays. This position formulates, implements and evaluates patient care management according to practice guidelines. Assists individuals/families to mobilize strengths in dealing with well health maintenance, self-care skills, adaptation to illness and health promotion, death/grief and bereavement. This is a .79 FTE position, working 31.6 hours per week. It also includes Saturday (8am-12pm) Walk-In Clinic rotation. Responsibilities:- Assess patient to determine physical and emotional needs.
- Obtains complete medical history and physical data on patients.
- Orders appropriate diagnostic and therapeutic procedures as needed.
- Performs a complete physical exam and records findings.
- Collaborates with physicians in managing acute and long-term medical needs of
- Posts tests and examination results.
- Performs procedures approved within scope of practice and as instructed by
- Delegates care and makes appropriate referrals to specialties.
- Respects physical privacy of patients.
- Interviews and advises patients regarding health and illness prevention.
- Provides patient education.
- Instructs patients regarding medications and treatment instructions.
- Establish patient specific goals that are consistent with the overall plan of
- Recommends community resources to meet patient and family needs.
- Communicates changes in client status with provider and/or family members.
- Modifies, implements and evaluates individualized teaching plan in order to
- Gives pertinent and timely report to co-workers.
- Ability to utilize electronic medical record effectively.
- Documents accurately and legibly.
