Job Overview Angels of Care Pediatric Home Health is a clinician‑owned home health agency serving children and young adults with special needs. We are looking for certified Physical Therapists (PT) (part‑time and full‑time) to provide home‑based rehabilitation services. Pay Pay Range: $63,000 – $114,000 + $1,000 Sign‑on Bonus. Job Description A certified Physical Therapist will implement treatment programs for pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays by planning and administering physical therapy services in the home and community. Requirements Texas State PT license Current CPR certification Minimum 1 year experience preferred Responsibilities Provide high‑quality care and meet the needs of patients and families by performing evaluations, interpreting assessment results, developing goals, and creating and implementing physical therapy treatment plans in collaboration with the physician. Assist pediatric patients in developing or regaining physical, neurological, cognitive and/or social/emotional functioning, improving independence by implementing skilled interventions. Educate, coach, and empower caregivers and families to incorporate therapeutic activities into the child's daily routine. Coordinate with referral partners to provide services in accordance with the physician's order. Ensure continuation of the therapeutic plan post‑discharge by designing and instructing patients, families, and caregivers in home exercise programs. Document patient care services and care coordination in an electronic medical record system. Maintain patient confidentiality.
