Position Overview

St. Elara Medical Center is actively seeking a dedicated and compassionate Physical Therapist to join our team in Waldwick, New Jersey. Work Environment

This position is primarily onsite within various assisted living facilities, allowing you to engage directly with residents in their living environment. You will work Monday to Friday from 9 AM to 5 PM, ensuring you have a balanced schedule while providing necessary support to our residents. The work environment is designed to promote collaborative efforts between physical therapists, orthopedic physicians, occupational therapists, and chiropractic doctors, emphasizing a holistic approach to healthcare.

Key Responsibilities

• Deliver on-site therapy services that cater specifically to residents within affiliated assisted living communities, fostering an individual approach to each patient's rehabilitation.
• Conduct comprehensive evaluations and collaboratively design orthopedic-informed treatment plans tailored to each resident's unique needs.
• Coordinate effectively with facility nursing staff and caregivers, ensuring a seamless integration of care among our multidisciplinary team.
• Provide education to residents and their families regarding safe mobility practices, home exercise programs, and wellness strategies, empowering them to take charge of their health.
• Utilize mobile electronic medical record (EMR) tools for efficient, HIPAA-compliant documentation, streamlining administrative processes in a patient-centered manner.
• Serve as a vital link between the assisted living facility and our multidisciplinary clinic, facilitating referrals and follow-up care to foster better health outcomes.

Patient Population / Client Focus

You will primarily work with older adults in assisted living settings, addressing a range of orthopedic conditions that are prevalent in this population. Our patients often face mobility challenges, chronic pain conditions, and post-surgical recoveries, making it imperative for our therapists to deliver personalized care that enhances their quality of life and independence.

Qualifications

• A valid Physical Therapist license in the State of New Jersey or an eligibility to obtain licensure.
• A minimum of two years of relevant clinical experience, particularly in outpatient settings, geriatrics, or assisted living environments.
• Strong communication and interpersonal skills that enable you to build rapport with patients and collaborate effectively with healthcare teams.
• Clinical knowledge and understanding of orthopedic conditions common among the aging population.
• A proven ability to work independently in a residential setting while delivering high-quality, patient-focused care.

Preferred Experience

Compensation and Benefits

We offer a competitive annual salary in the range of $90,000 to $100,000, commensurate with experience and qualifications. Our benefits package includes:
