Position Overview
We are seeking a dedicated Critical Care Clinical Pharmacist to provide expert pharmaceutical care to critically ill patients within a dynamic intensive care environment. This role involves close collaboration with medical and hospital staff during patient care rounds to ensure optimal medication management and patient safety.
Key Responsibilities
- Maintain a safe and clean work environment, adhering to unit-based safety and infection control protocols.
- Deliver cost-effective, evidence-based pharmaceutical care focused on critically ill patients, primarily within the ICU.
- Communicate medication order changes with prescribers, document updates accurately, and inform relevant team members.
- Collaborate with clinical dietitians to evaluate and optimize parenteral nutrition orders.
- Provide pharmacokinetic and clinical consultations as needed.
- Monitor, document, and report adverse drug reactions and medication errors, contributing to prevention strategies and process improvements.
- Offer drug information and intravenous compatibility guidance to the ICU team, utilizing regional poison control resources when appropriate.
- Promote formulary use and coordinate procurement of non-formulary medications when necessary.
- Educate ICU team members and pharmacy staff on drug therapy-related topics.
- Document clinical activities including disease-state management, pharmacotherapeutic monitoring, and education.
- Serve as a liaison among pharmacy, nursing, and medical staff to update and educate on drug-related policies and procedures.
- Participate in hospital committees addressing critical care pharmacotherapy issues.
- Work with Clinical Manager on quality assurance and clinical pathway development to enhance pharmaceutical care.
- Support pharmacy operations as needed.
- Develop and conduct targeted drug programs and usage evaluations, reporting findings to the Pharmacy and Therapeutics Committee.
- Actively engage in multidisciplinary critical care and code blue team rounds.
- Stay current with primary and tertiary references relevant to critical care pharmacotherapy.
- Maintain ACLS certification and respond to resuscitation events during scheduled shifts (excluding Emergency Department events).
Qualifications
- Minimum of one year inpatient pharmacy experience, primarily in a critical care setting.
- Familiarity with Joint Commission standards.
- Ability to contribute to cost-saving initiatives and optimize patient medication care.
- Post Graduate Year 1 (PGY1) residency preferred; Post Graduate Year 2 (PGY2) critical care residency highly desirable.
Education and Licenses
- Doctor of Pharmacy (PharmD) degree required.
- Current state pharmacy license required.
Certifications
- Board Certified Pharmacotherapy Specialist (BCPS) preferred.
- Current Basic Life Support (BLS) certification required upon hire.
- Current Advanced Cardiovascular Life Support (ACLS) certification required upon hire.
Successful candidates must pass a physical exam, drug screening, and background checks prior to employment.
