Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience with Home Care Home Base EMR preferred.
• Productivity expectation: 30 visits per week for registered therapists.
• Orientation required: 5 days.
• Ability to travel within the region as needed and provide care in patient homes.
• Reliable transportation and valid driver’s license for home health travel.
• Previous home health experience preferred. Candidates new to home health will be considered.

Preferred Qualifications

• Prior home health caseload management experience.
• Experience working with medically complex or post-acute patients.
• Familiarity with productivity tracking and time management in home health settings.

Assignment Details

• Schedule: Monday through Friday.
• EMR: Home Care Home Base.
• Orientation: 5 days.
• Productivity expectation: 30 visits per week for registered therapists.
• Expected mileage: varies. Mileage reimbursed at IRS standard rate.

Impacting Patient Care

Provide skilled physical therapy services in patient homes throughout Springfield and surrounding communities. You will evaluate, treat, and document interventions that improve function and safety. You will coordinate with interdisciplinary teams and contribute to continuity of care for post-acute and chronic patients. Maintain compliance with regulatory and documentation requirements while delivering patient-centered care.
