Join a respected healthcare team as an RN Care Manager! You'll coordinate patient care plans, assess healthcare needs, collaborate with medical teams, and help ensure patients receive the right level of care at the right time. This role focuses on improving patient outcomes through effective care coordination, quality management, and safe transitions throughout the healthcare journey.

 

Shifts

• Day Shift
• 8-hour shifts

 

Responsibilities

• Coordinate patient care activities and communicate with multidisciplinary teams
• Assess patient conditions and develop appropriate nursing care plans
• Monitor patient progress and update care plans as necessary
• Provide education and support to patients and families regarding treatment plans
• Ensure compliance with healthcare regulations and institutional policies
• Participate in quality improvement initiatives and documentation processes
• Facilitate transitions of care and coordinate discharge planning

 

Requirements

• Active Kansas Registered Nurse (RN) License
• Associate Degree in Nursing (ADN) or Nursing Diploma from an accredited nursing program
• BSN preferred
• Minimum 2 years of acute care Med/Surg nursing experience
• Strong clinical assessment and care coordination skills
• Excellent communication and organizational abilities

 

Salary-

• Starting at $30.11+/hour (based on education and experience)
