Riverwood Health and Rehab Center offers an exceptional environment for healing, recovery, and daily living in a warm, home-like setting. Social Services Director Duties & Requirements:
- Evaluating clients to determine their needs and the support options that can help them
- Creating individualized care plans for each client
- Referring clients to community resources to help their specific situations
- Collaborating with other care providers to coordinate services
- Monitoring clients
- Maintaining records of treatments and services
- Providing talk therapy
- Responding to crisis situations
- Additional duties & requirements as requested by job description, Administrator, and Regional Support Team
