Job Description: Championing Patient-Centered Nursing Personalized Care: Deliver skilled nursing care, including medication administration, wound care, and cardiac/respiratory support. Care Coordination: Collaborate with healthcare teams to implement individualized care plans. Regulatory Compliance: Maintain accurate documentation and adhere to all agency and regulatory standards. Supervision and Training: Guide and mentor LPNs and Home Health Aides to ensure the highest quality of care. Candidate Profile: A Compassionate and Skilled Professional Credentials: Current RN license in Virginia with experience in public health, home health care, or hospital settings. Clinical Experience: At least 1 year of hands‑on nursing experience, preferably in skilled care or private duty. Leadership and Independence: Confidence in managing care plans and working autonomously in a home setting. Empathy and Communication: Strong ability to connect with patients and families while maintaining professionalism.
