Honor Community Health seeks a Full Time Targeted Case Manager to coordinate access to medical, behavioral health, and social services within a Certified Community Behavioral Health Clinic. Reporting to the Director of Care Management, you will help clients sustain recovery and independence in the community. The role emphasizes collaboration, outreach, and connecting individuals to resources, with standard hours Monday through Friday, 8:30am to 5:00pm, and a commitment to serving underserved #J-18808-Ljbffr Honor Community Health
