Licensed Vocational Nurse (LVN) – Full-Time AM ShiftFacility: Oakwood Gardens Care Center Shift: AM Shift Salary Range: $28.00 – $30.00 per hour (BOE)General PurposeThe Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs). Care is delivered in accordance with current federal, state, and local regulations, facility policies, and under the direction of the Director of Nursing (DON) or Nurse Supervisor to ensure the highest quality of resident care.Essential Duties & ResponsibilitiesProvide direct nursing care in accordance with resident assessments and care plansSupervise and support CNAs to ensure quality care deliveryAdminister medications and treatments as ordered by physiciansAccurately document nursing care, medication administration, and resident statusMonitor residents for changes in condition and report findings promptlyParticipate in admissions, transfers, and discharges as assignedEnsure compliance with infection control, safety, and sanitation standardsCommunicate effectively with residents, families, physicians, and interdisciplinary team membersAssist with restorative and rehabilitative nursing programsRespond appropriately to emergencies and resident needsMaintain resident confidentiality and comply with HIPAA regulationsSupervisory ResponsibilitiesProvides direct supervision to CNAs during assigned shiftEnsures staff adhere to policies, procedures, and scope of practiceQualificationsEducation & ExperienceGraduate of an approved LVN/LPN programLong-term care or skilled nursing experience preferredLicenses & CertificationsCurrent, active, and unencumbered LVN license in the State of CaliforniaCurrent CPR certificationKnowledge of PCC preferredSkills & AbilitiesStrong clinical, communication, and leadership skillsAbility to work independently and as part of a teamAbility to handle fast-paced and emotionally demanding environmentsProfessional, compassionate, and resident-centered approach to carePhysical DemandsAbility to stand, walk, bend, and move throughout the shiftAbility to lift, push, pull, and assist residents (up to 25 lbs routinely; more with assistance)Ability to respond to emergencies, including resident evacuation if neededWork EnvironmentSkilled nursing facility environmentExposure to residents, visitors, and healthcare equipmentNoise level generally low to moderateReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsWe are an equal opportunity employer.
