Job Opportunity: Registered Nurse Case Manager

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.

Key Responsibilities

Manage patient care in hospice settings

Coordinate with healthcare teams

Provide leadership and support to nursing staff

Territory/Driving Expectation: Greesnburg and 15-25 miles +/- in a radius surrounding the community

Schedule (Including any on call/weekends): 6 units daily; 30 units a week; 8 AM 5 PM, Mon-Fri schedule. 1 weekend a month; 1 evening of on-call per week.

Travel

Local travel required covering multiple counties.

Qualifications Required

High School/GED

Hospice (1+ years)

Management (1+ years)

1+ years of experience in ANY of the following: Home Health Care Epic Home Health Home Health

Valid Registered Nurse (RN) in the state of Pennsylvania (PA)

ALL of the following valid licenses/certifications: American Heart Association Basic Life Support (AHA BLS) American Heart Association Advanced Cardiac Life Support (AHA ACLS)
