RN: Case Manager

Job Title - RN: Case Manager Shift : Monday to Friday, 8 AM to 5 PM (Weekend on-call rotation) Productivity: 30 points per week License Required : RN License in Indiana or Compact License accepted Mileage Reimbursement : $0.46 per mile Coverage Areas : Clark, Floyd, Harrison, Crawford, Scott, Washington, Jefferson, and part of Orange County. May also help in other areas for PTO coverage, call-ins, and admissions. Required Skills : Wound care, IV care, drain care, G-tube care, and patient education. HCHB experience preferred. Primary Responsibilities: Complete assessments for high-risk patients Create and follow individualized care plans Coordinate with doctors, social workers, and care teams Support patients with non-medical needs like housing and transportation Manage care transitions and discharge planning Keep accurate patient and care records Promote health and prevent illness Protect patient privacy and confidentiality Qualifications: Associate’s or Bachelor’s Degree in Nursing Active and unrestricted RN license Strong decision-making and medical assessment skills Case Management certification preferred Employment Type: Full Time Salary: $55 - $60 Annual Bonus/Commission: No
