Social Worker - Case Management Southwest Medical Center is seeking a Medical Social Worker (Care Coordination) to join our Utilization Review and Case Management Department. This position offers an exciting opportunity to combine professional medical social work with care coordination and discharge planning while working as part of a collaborative interdisciplinary healthcare team. The Medical Social Worker (Care Coordination) serves as an advocate for patients and families by helping identify barriers to care, coordinating community resources and post-acute services, and promoting safe, timely transitions throughout the continuum of care. Qualifications (Required) Bachelor's degree or higher in Social Work (BSW or MSW) from an accredited college or university. Current Kansas social work license in good standing appropriate for the assigned duties (LBSW, LMSW, LSCSW, or other applicable social work license recognized by the Kansas Behavioral Sciences Regulatory Board). Qualifications (Preferred) Hospital social work experience. Care coordination or case management experience. Behavioral health. Skilled nursing, long-term care, home health, hospice, or other healthcare settings. Key Responsibilities Complete psychosocial assessments. Identify barriers affecting treatment, recovery, and discharge. Connect patients and families with community resources and financial assistance. Coordinate referrals for home health, hospice, rehabilitation, skilled nursing facilities, behavioral health services, durable medical equipment, and other post-acute services. Assist patients with advance directives. Participate in interdisciplinary care planning and discharge planning. Collaborate with physicians, nurses, therapists, Patient Financial Services, and community agencies. Promote safe, timely, patient-centered transitions of care. Maintain timely and accurate documentation in the electronic medical record. #J-18808-Ljbffr
