Patient Care CoordinatorThe Patient Care Coordinator is responsible for managing the day-to-day coordination of caregiver schedules and ensuring all client cases are properly staffed and maintained. This role operates at the center of operations, balancing client needs, caregiver availability, and service continuity in a fast-paced home care environment. The Coordinator owns case coverage, communication, and problem resolution, ensuring no gaps in care. This position plays a critical role in client satisfaction, caregiver retention, and the overall stability of a new and expanding office.Key Responsibilities:Conduct new client welcome calls and assist with onboarding and service setupManage and maintain active client cases, ensuring all shifts are fully staffed and coveredMonitor authorizations, schedules, and service hours to ensure compliance with approved care plansTrack and resolve missed visits, EVV exceptions in LTSS, and scheduling discrepancies in a timely mannerAssist with incident reporting, hospitalization updates, and other compliance-related documentationParticipate in on-call rotation and provide after-hours coordination support when requiredMatch caregivers to client needs based on availability, location, and skill setMonitor open cases and take immediate action to fill unstaffed or last-minute coverage needsMaintain accurate scheduling records and updates in internal systemsRespond to client concerns and escalate issues when necessaryTrack and follow up on attendance, reliability, and caregiver performance concernsSupport onboarding coordination by aligning newly cleared caregivers with open casesPerformance Expectations:Maintain full coverage of assigned client cases with minimal gapsRespond quickly to call-outs and staffing issues with same-day resolutionEnsure accurate and up-to-date scheduling records at all timesMaintain strong communication with caregivers and clientsContribute to client satisfaction and caregiver retention through consistent coordinationDemonstrate ownership of cases and accountability for outcomesQualifications:Associate or Bachelor's degree in Healthcare Administration, Business, or related field preferredExperience: 1+ year of experience in scheduling, coordination, or operations (home care preferred)Experience managing multiple schedules or high-volume coordination tasksStrong communication and problem-solving skillsAbility to work in a fast-paced, high-pressure environmentCore Competencies:Strong organizational and time management skillsAbility to multitask and prioritize under pressureHigh sense of urgency and accountabilityClear and professional communicationProblem-solving and decision-making abilityAttention to detail and accuracyReliability and consistency
