Registered Nurse - Telehealth Home Care (ACH)
Mayo Clinic
Mayo Clinic is seeking an on-site Registered Nurse for Advanced Care at Home (ACH) in Jacksonville, FL. The ACH RN will perform patient assessments, education, medication verification, and discharge planning while coordinating with a multidisciplinary team through telemedicine and on-site involvement as needed.
Candidates should have an RN license and BLS, with preference for those with prior RN experience.
