Key Responsibilities:

• Provide high-quality nursing care to long-term care patients in accordance with facility protocols.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Maintain accurate and timely documentation of patient care activities using EMR systems.
• Administer medications, IV therapy, and treatments as prescribed, ensuring patient safety.
• Execute patient assessments and respond promptly to changes in patient conditions.

Qualifications:

• Bachelor of Science in Nursing (BSN) or associate degree in nursing from accredited institution.
• Ohio RN License.
• BLS (Basic Life Support) certification.
• IV certification.
• Minimum 2 years of skilled nursing facility experience.
• Proven experience in long-term care settings.
• Strong clinical assessment skills.
• Ability to work rotating 3x12 shifts.
• Excellent communication and teamwork skills.
• Ability to handle call-back duties and rotating weekends.
• Physical ability to stand and perform patient care duties for extended periods.
• Competent in EMR usage and documentation.
