Job Title: Clinical Nurse Profession: RN Specialty: Outpatient Duration: 9 months Shift: 9AM - 5PM Hours per Shift: 7.5 Experience: Minimum 2 years of experience in ambulatory care and cardiology License: NYS RN License Certifications: BLS Must-Have: Bachelor of Science in Nursing (BSN) required. Cardiology experience and IV skills required. Non-invasive cardiology experience highly preferred. Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Applies the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care, and delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease while anticipating needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing, effectively allocating material resources. Role models the tenets of Relationship Centered Care through various facilitated actions. Considers patients' values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Uses evidence-based practices to improve understanding of patients' perceptions of care. Initiates service recovery for patients and family members raising concerns and escalates as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on evidence-based practices and institutional protocols. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high-risk behaviors providing a safe environment. Escalates potential safety hazards or gaps from best practice following established protocols. Utilizes institutional information technology for documenting nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned and coordinates with management to ensure efficient supply utilization. Escalates material or staffing resource deficits to management as necessary. Documents accurately and thoroughly in compliance with care standards and regulations. Facilitates patient throughput on a shift basis. Contributes to the environment of care supporting clinical colleagues, patients, and their families. Advances clinical competence in nursing practice and communicates with styles that demonstrate empathy and respect. Incorporates ethical principles into decision-making for patients and families. Encourages and demonstrates a spirit of lifelong learning and innovation. Acquires knowledge and skills relative to the role and patient population needs. Participates in shared decision-making through practice councils and unit initiatives.
