Requirements Candidate CVs need to have month & date on their work experience w/all gaps in employment accounted for. Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD Monday-Friday 8 am - 5 pm Mix of peds/adults A decision is typically made within 7-14 days We will also accept DO BE will be considered if truly BE (5 years or less since residency) Active DEA needed before start Credentialing takes 30-90 days The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Covid-19 Vaccine no longer required HCO & Vendor agree to a 30 day notice for ending all assignments.
