Schedule: Sun-Thur or Tu-Sat

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 30 per week

Requirements:

• License as required for the role

Roles & Responsibilities:

• Provide patient care and support in accordance with accepted practices.
• Collaborate with a multidisciplinary team to support patient wellness.
• Maintain accurate and timely documentation of patient interactions.
• Assist in the development and implementation of patient care plans.
• Educate patients and families about health management and resources.
