RN Case Manager: Discharge & Care Coordination

AdventHealth in Daytona Beach, FL seeks a RN Case Manager to actively participate in rounds, evaluate discharge needs, and educate patients and families on care transitions. The role emphasizes coordinating post-acute services, discussing resources with Payors, and ensuring timely, safe discharges across inpatient, observation, and ED settings.
