Why Join Victorian Post Acute?* **$20.00–$25.00+ per hour, DOE*** **Full-Time position*** Medical, dental & vision benefits* **401(k) retirement plan*** **Paid Time Off (PTO)*** Employee recognition and rewards* **Bonus opportunities*** Training and professional development* Career growth opportunities* Supportive and collaborative work environment* Convenient San Francisco location with **ample parking** What You’ll DoAs a Social Services Assistant, you will play an important role in supporting residents and their families and helping ensure a positive experience throughout their stay. **Responsibilities include:*** Build positive, meaningful relationships with residents and their families.* Provide compassionate support for residents’ social, emotional, and psychosocial needs.* Assist with care planning and help identify goals that promote independence, dignity, and quality of life.* Provide emotional support to residents and families experiencing illness, disability, adjustment, grief, or loss.* Assist with **admissions, transfers, and discharges**.* Participate in discharge planning and help connect residents with appropriate community resources.* Interview residents, guardians, and sponsors to obtain required admission information.* Prepare and organize admission packets, including resident rights, privacy notices, and admission agreements.* Maintain accurate resident records and identification information.* Maintain accurate bed availability records.* Collaborate with nursing, rehabilitation, administration, and other interdisciplinary team members.* Maintain resident confidentiality and follow facility policies and applicable privacy requirements. What We’re Looking ForWe’re looking for someone who is **compassionate, organized, dependable, and genuinely enjoys helping others.** **Qualifications:*** High school diploma or equivalent required.* Previous experience in healthcare, skilled nursing, senior living, or social services preferred.* **PointClickCare (PCC) experience is a plus, but not required.*** Excellent communication and interpersonal skills.* Strong organizational and documentation skills.* Ability to manage multiple priorities in a fast-paced environment.* Ability to work effectively with residents, families, staff, and community resources.* Empathetic, patient, professional, and resident-focused approach.
