Key Responsibilities Provide high‑quality nursing care in patients’ homes in accordance with Family First Homecare policies, state Nurse Practice Acts, and all applicable regulations. Maintain an updated clinical record for each patient, meeting all required documentation deadlines. Evaluate the effectiveness of nursing services to patients and families on an ongoing basis. Consult with physicians regarding changes to patient care plans and document all alterations and reports to the nurse supervisor. Mentor and coach new employees during patient‑specific orientations. Educate patients and caregivers on topics related to patient care needs. Participate in the Quality Assurance, Performance Improvement (QAPI) program as requested. Continuously strive to improve nursing care skills. Attend and complete all mandatory training and in‑services assigned. Submit required personnel file documentation in a timely manner. Adhere to compliance, ethics expectations, patient and family confidentiality, and professional communication standards. Perform other duties as assigned. Role Qualifications LPN/LVN nursing degree. Unencumbered LPN/LVN license in the state of employment or an active multi‑state license through the Enhanced Nurse Licensure Compact. At least 1 year of demonstrated experience. Valid driver license with proof of insurance (when required). Clear background screening. Valid CPR card/certificate with in‑person instructor‑led hands‑on skills check. Clear HepatitisB and TB screening (state requirements apply). Preferred Qualifications State‑specific knowledge and certifications (detailed by state below). State Specific Requirements Florida Valid driver license with proof of insurance not required. Staff prohibited from driving patients. HepatitisB screening not a prerequisite for hire; completed during onboarding. Illinois Pennsylvania Additional background screening for child abuse and FBI check. Must complete TB screening with 2‑step PPD or provide proof of 2‑step PPD administration within the last year. Texas Other states: check applicable regulations. Key Competencies Ability to read and communicate in English, both verbally and in writing. Working Conditions / Equipment Use Work performed in patient homes; no substantial exposure to adverse environmental conditions. Training may occur in an office setting. Must be able to lift up to 35 pounds unassisted. Frequent use of office equipment including telephone, computer, and printer.
