Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home health experience
• OASIS experience
• HCHB experience
• Physical Therapist License
• Bachelor's Degree

Assignment Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• No on-call or weekend requirement
• Coverage area includes Shelby County, eastern Jefferson County, and Spencer County
• Responsibilities include SOCs, ROCs, routine visits, supervisory visits, and re-assessments
