Nurse Practitioner

Position: Nurse Practitioner (Advanced Practice Provider) Title: Nurse Practitioner, Advanced Practice Provider at Oak Street Health. The role provides effective, equitable value‑based primary care to adult Medicare patients. Core Responsibilities Provide exceptional primary care including office visits for routine and acute issues. Administer annual wellness visits and health risk assessments with holistic documentation. Coordinate care with other providers, specialists, testing facilities, and agencies. Lead population‑health activities (e.g., ensuring eligible females receive breast‑cancer screening every two years). Assist care team with phone triage, outreach, patient education, and home visits as needed. Participate in Oak Street Health promotional activities. Other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family NP, Adult‑Gerontology, Primary Care NP, Adult NP, Gerontological NP. Active, non‑probationary state NP license. Active DEA license. US work authorization. Preferred Qualifications Experience in primary‑care settings (internal medicine, geriatrics, or family medicine). Understanding of HCC documentation, ICD‑10 coding, and Health Risk Assessments (HRAs). Team‑oriented mindset and collaboration across functions. Interest in an empirically‑guided population‑health model. Bi‑lingual proficiency in applicable areas. Pay Range The typical pay range is $87,035.00 – $187,460.00 annually.
