Case Manager

We are seeking a dedicated and skilled Case Manager to provide exceptional patient care through the collaboration, development, implementation, revision, and reporting of a comprehensive case management program. The Case Manager will act as a liaison between patients, families, nurses, physicians, multidisciplinary teams, and healthcare benefactors to optimize patient outcomes.

Job Summary

The Case Manager will coordinate a continuum of care for a defined patient population, ensuring that patients and families navigate efficiently throughout the hospitalization process. This role requires a strong commitment to supporting the healthcare team and ensuring quality patient care.

Essential Functions

• Lead the coordination of patient care across various disciplines within the care team, ensuring the appropriateness and timeliness of services provided.
• Oversee the interdisciplinary care plan, aligning it with the patient's clinical course, continuing care needs, and applicable covered services, monitoring diagnostic testing, treatments, and procedures.
• Engage with physicians regarding resource utilization, consultations, treatment plans, estimated length of stay, and discharge planning.
• Focus on complex cases, including frequent emergency department utilizers, chronic pain patients, substance abuse patients, and those experiencing homelessness.
• Collaborate with acute care case managers to ensure ongoing care is appropriate and effective.
• Coordinate patient transfers to tertiary care centers, managing the transfer of necessary patient information to ensure continuity of care.
• Provide oversight and collect data to ensure regulatory and accreditation compliance.
• Conduct focused reviews of previous and current admissions for frequent emergency department visitors.
• Perform other job-related duties as assigned.

Education and Work Experience

• Bachelor's Degree in Nursing (BSN) is preferred.
• Preferred experience in a care management role.

Licenses/Certifications

• Registered Nurse (RN) licensure in the state of practice is required.
• Case management certification is preferred.
• Basic Life Support (BLS) certification from an approved vendor as per policy is required.

Organizational Requirements

Commitment to the safety and wellbeing of all associates and patients is paramount. Candidates must possess at least two years of recent experience in the field, current BLS certification, and a valid nursing license as per state requirements.
