What You'll Do:

As a physical therapist in our long term care facility, you will evaluate, rehabilitate, and improve the physical functioning of patients with diverse disorders, including wound care. Your role involves developing personalized care plans, administering treatments such as manual therapy, therapeutic exercises, modalities, and assistive device support. You will monitor patient progress, adjust treatments accordingly, and collaborate closely with a multidisciplinary team. Providing education and counseling to patients and their families, documenting care meticulously, and participating in team meetings are key components of your daily responsibilities. Your expertise will help patients regain independence and achieve their health goals.

Required Skills:

• Valid state licensure as a physical therapist
• Proven experience in physical therapy, preferably in long-term care or rehabilitation settings
• Strong assessment and clinical decision-making skills
• Knowledge of evidence-based practice and standardized assessment tools
• Excellent communication skills for effective collaboration with patients, families, and team members
• Ability to document accurately in electronic health records
• Ability to supervise and support rehabilitation aides, students, or assistants

Nice to Have Skills:

• Experience with wound care management
• Familiarity with DME procurement and supply management
• Prior experience working in regulatory environments and understanding compliance standards
• CPR or other relevant certifications

Preferred Education and Experience:

• Bachelor's or master's degree in physical therapy from an accredited program
• At least 1+ year of clinical experience in a rehabilitation or long-term care setting
