Aptive Resources, in partnership with Valor Healthcare, is hiring a Registered Nurse Care Manager to provide care coordination and chronic disease management within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award. Location: On-Site in Yreka, CA. Schedule: Mon–Fri, 7:30am–5:30pm; clinical hours 8:00am–4:30pm.
