HIM Assistant

About the Role The HIM Assistant is responsible for assembling patient information into patients’ electronic medical record in an accurate and timely manner. The primary functions of the position revolve around the accurate scanning and routing of all incoming documents, quality analysis of the contents of the records, chart completion, and processing release of information requests for all USUP locations. The HIM Assistants will work closely with the EMR Specialists to continuously evolve and improve processes. Responsibilities Properly identify, accurately scan, and route all incoming electronic documents and internal documents, ensuring they are correctly filed in the patient’s chart and electronically delivered to the appropriate parties. Maintain all patient information according to the established patient confidentiality policy. Handle incoming new patient referrals, create new patient charts, retrieve records and ensure all necessary records are obtained and accurate. Answer phones and handle new patient self-referrals, urgent care and hospital follow-ups, and all incoming patient record request phone calls. Monitor, process, and reconcile all communication tanks, including direct messaging, HL7, and the exception tank, for routing and importing, as well as entering lab results in a timely and accurate manner. Obtain office medical records requests to retrieve records for hospital follow-ups via hospital portals, urgent routing for incoming triages and patient appointments within 24 hours. Collect, evaluate, scan, and route all documents from all locations, ensuring proper QA by the HIM Supervisor. Complete daily document distribution, transmitting provider office notes as outlined in the patient DDCC. Monitor and accurately handle all outlook results, ensuring specialty testing reports are in the correct patient’s chart. Receive and process medical record requests for all locations according to the Release of Information policy and procedure; track releases within Medent and invoice eligible parties for payment of records prior to release, posting processing fees per the USUP policy. Cross‑train and rotate duties as assigned, performing all other duties as required. Qualifications Associate Degree required. Minimum of 2 years’ experience in HIM. Prior use of Medent systems preferred. Reasoning ability: solve practical problems and interpret instructions in various forms. Computer skills: thorough knowledge of computer information systems. Physical Demands The physical demands include standing, walking, sitting, using hands to finger, handle, or feel, reaching with hands and arms, stooping, kneeling, crouching, or crawling, and talking or hearing. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Work Environment The role operates in a professional office environment using standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
