Job Summary

Physical Therapist, Outpatient. Provides comprehensive outpatient physical therapy services, including patient evaluations, individualized treatment planning, treatment implementation, and progress toward discharge. The role also includes patient and family education, clinical documentation, care coordination, and participation in professional development and quality improvement activities.

Job Details

• Schedule: Day shift, 8 hours per shift, 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification; active and applicable state professional license.
• Additional Information: Evaluates assigned patients and develops, implements, and modifies treatment plans based on patient needs, goals, and physician direction. Provides discipline-specific therapy services, designs functional treatment programs, documents evaluations and treatment activities accurately and timely, delivers patient and family education, supports student learning when assigned, and participates in continuing education and quality improvement initiatives.
