Comprehensive outpatient primary care Preventive medicine and wellness visits Diagnosis and management of acute and chronic illnesses Care for patients across the lifespan Optional minor office procedures based on training and experience Team-based care model with strong clinical support Opportunity to build long-term patient relationships Key Responsibilities

Perform comprehensive patient assessments, physical examinations, and preventive health evaluations Diagnose and manage acute and chronic medical conditions across the lifespan Develop individualized treatment plans focused on improving patient outcomes Order, interpret, and communicate laboratory and diagnostic test results Prescribe medications and therapeutic interventions in accordance with Arizona regulations Provide patient education on disease prevention, chronic disease management, and healthy lifestyle choices Collaborate with physicians, nurses, behavioral health professionals, and support staff to coordinate patient care Perform minor in-office procedures based on training and credentialing, if desired Maintain accurate and timely documentation within the electronic medical record Participate in quality improvement initiatives and support organizational goals Qualifications

Required

Graduate of an accredited Physician Assistant or Family Nurse Practitioner program Current certification as a

PA-C

or

Family Nurse Practitioner (FNP) Eligible for Arizona licensure DEA registration (or ability to obtain) Strong communication and interpersonal skills Commitment to delivering compassionate, patient-centered care Preferred

Primary care or community health experience Passion for serving rural and underserved populations Cultural sensitivity and commitment to health equity New graduates are encouraged to apply Compensation & Benefits
