Our hospice team provides exceptional, patient-centered care—supporting individuals and families through one of life's most meaningful transitions.Key ResponsibilitiesProvide psychosocial support, counseling, and education to hospice patients and their loved ones.Conduct assessments to identify emotional, social, and financial needs.Collaborate with nurses, aides, chaplains, and physicians as part of an interdisciplinary care team.Assist families with advance care planning, community resources, and bereavement support.Advocate for patient rights, dignity, and preferences.Maintain accurate, timely documentation in accordance with agency and regulatory standards.QualificationsLicensed Social Worker (LSW/LCSW/LMSW or equivalent state licensure).Experience in hospice or healthcare setting preferred.Exceptional communication, counseling, and advocacy skills.Compassion, flexibility, and the ability to support patients and families through emotional transitions.Valid driver's license and reliable transportation required.If you're looking for a career that's transformative for you and those you serve, then you've come to the right place.
